Visiting Nurse Specialized Adult Day Care program is designed to provide assistance for adults with Alzheimer’s disease, or other related dementias in need of supervision and social interaction. The program offers respite care in a safe, structured setting.

Eligibility

Diagnosis of Alzheimer’s disease or other related forms of decreased mental abilities (dementia)

Fee

Patients are assessed on a sliding fee scale based on patient/family taxable income.
