Palliative Care

Palliative care focuses on improving the quality of life for individuals and their loved ones while managing a chronic illness. The goal of this specialized care is to enhance the quality of life by managing the symptoms associated with chronic disease, provide support, and assist with planning care for the future when the disease progresses.

The palliative care team includes nurses, therapists, home health aides, social workers, chaplains, and nurse practitioners. This interdisciplinary team works in partnership with your physician to design a plan of care that manages the symptoms associated with chronic disease and offer a support system to help individuals remain as active as possible.
